Ankylosing spondylitis (AS) is a form of chronic inflammation of the spine and sacroiliac joints. Chronic inflammation in these areas causes pain and stiffness in and around the spine. Over time, chronic spinal inflammation may lead to a complete cementing together (fusion) of the vertebrae, a process called ankylosis. Ankylosis leads to loss of mobility of the spine and the condition known as bamboo spine.
It is important to note that the course of ankylosing spondylitis varies widely from person to person. So too can the onset of symptoms. Although symptoms usually begin to appear in late adolescence or early adulthood (ages 17-35), symptoms can occur in children or much later. Usually the first symptoms of AD are frequently the pain and stiffness in the lower back and buttocks, which is gradually over a few weeks or months. At first, just a discomfort can be felt by one side or alternating sides.
The pain is usually dull and diffuse, not localized. This pain and stiffness is usually worse in the morning and at night, but can be improved by a warm shower or light exercise. Moreover, in the early stages of AD, mild fever may occur, loss of appetite and malaise. The pain usually becomes persistent (chronic) and is felt on both sides, usually persists for at least three months. In the course of months or years, the stiffness and pain may spread to the spine and neck. The pain and tenderness extends to the ribs, shoulder blades, hips, thighs and heels is also possible. Men are more affected than women by a ratio of 3:1. As can also be associated with ulcerative colitis, Crohn's disease, psoriasis and Reiter's disease.
